Insurance Plans and Billing Policies

In order to be available to treat you, we participate in most major insurance plans.  The following list is for informational purposes. Due to changes in insurance plans, our participation in the the following plans could change.  Please call us directly to verify coverage by your specific insurance plan.

If your insurance requires prior authorization or a referral is required, please contact your primary care physician before scheduling your appointment.  If you are referred to us for Mohs micrographic surgery, it is important that you tell your primary care physician’s staff the following codes:  Procedure (CPT) code 17111 or 17313, and Diagnosis (ICD-10) code C44.XXX.

Expenses that are not covered include cosmetic procedures, your deductible, and any co-pay or Out of Pocket amounts.

These fees will be collected at the time of service.  Payments may be paid by cash, check or credit card.

If prior authorization or a referral is required please contact your primary care physician (PCP) before scheduling your appointment. You are having a Mohs surgical procedure. It will be performed in the office, and any reconstruction (if needed) will be performed as an outpatient procedure in the office as well.

It is important that the paperwork be sent from your PCP before we can schedule your appointment.  Please contact our Procedure Scheduling Coordinator for the appropriate ICD-10 and CPT code information.

We do not want anyone to be denied medical care because of an inability to pay.  If you have difficulties understanding or paying your bill, we encourage you to discuss any issues with out billing office. The billing staff can be reached at (210) 558-6234.
